Total vs Partial Knee Replacement — Which Is Right for You?

Total Knee Replacement (TKR)

Total knee replacement resurfaces all three compartments of the knee joint, the medial tibiofemoral, lateral tibiofemoral and patellofemoral compartments, with metal alloy and high-density polyethylene components. It is the appropriate intervention for patients with advanced tricompartmental arthritis or significant joint deformity, and it is the most commonly performed form of knee arthroplasty. National registry data confirm excellent long-term outcomes in the vast majority of patients.

Unicompartmental (Partial) Knee Replacement (UKR)

Where arthritis is confined to a single compartment of the knee, most frequently the medial tibiofemoral compartment, and the remaining compartments and cruciate ligaments are structurally intact, partial knee replacement is a less extensive alternative. UKR preserves more native bone and soft tissue, typically allows a faster post-operative recovery and may better replicate the natural kinematics of the knee. Precise patient selection is essential; not all patients with predominantly medial symptoms are anatomically or biomechanically suitable.

The Treatments Pathway

Pre-operative consultation: Review of symptoms, clinical examination, standing weight-bearing radiographs and, where indicated, MRI or CT for Treatments planning. Full discussion of procedure, risks, recovery and expectations.

Pre-operative assessment: A nursing-led medical assessment including bloods, ECG, blood pressure, medication review and anaesthetic assessment, typically two to four weeks before Procedure.

Procedure: Performed under spinal anaesthesia, the preferred approach as it reduces blood loss, nausea and post-operative confusion, or general anaesthesia according to clinical and patient preference. TKR operative time is approximately 90 to 120 minutes. UKR is typically completed in under 60 minutes.

Inpatient care: Physiotherapy-led mobilisation commences on the day of Procedure. Patients undergoing TKR typically remain in hospital for two to three nights. UKR patients may be eligible for earlier discharge.
