Ganglions are the most common soft tissue swellings of the hand and wrist. They arise from the synovial lining of a joint capsule or tendon sheath and present as smooth, firm, fluctuant swellings that may appear quite suddenly and vary in size over time.

While a significant proportion resolve spontaneously, particularly in younger patients, many persist and become symptomatic. Where Do Ganglion Cysts Develop?

Ganglions are most frequently located at the following sites:

Dorsal wrist: The most common location — a smooth swelling on the back of the wrist, typically arising from the scapholunate ligament or the wrist joint capsule.

Volar wrist: On the palm side of the wrist, often near the radial artery. These require particular care at excision due to proximity to neurovascular structures.

Base of the fingers: Small ganglions arising from the flexor tendon sheath. Despite their small size, these can cause significant pain on gripping.

Dorsum of the foot: Less common but frequently symptomatic due to pressure from footwear.

Mucous cysts: A specific variant arising near the distal interphalangeal joint, often associated with underlying osteoarthritis of that joint.

Ganglion Treatment Options at Our Liverpool Clinic

Clinical Assessment

We examine the ganglion clinically, including assessment of range of motion, neurovascular status and the relationship of the cyst to adjacent structures. Where diagnosis is in question or Treatments planning is required, ultrasound imaging is arranged.

Aspiration (Needle Drainage)

Under local anaesthetic, a needle is introduced into the ganglion and the gelatinous fluid is aspirated. The procedure is quick, minimally invasive and requires no sutures. Recovery is immediate.

Aspiration is associated with a recurrence rate of approximately 50%. It is most appropriate when the patient prefers a Non-Invasive first approach, or where the ganglion is small, mildly symptomatic and the patient wishes to avoid Procedure.

Treatments Excision

The ganglion and its stalk — which connects the cyst to the underlying joint capsule or tendon sheath — are excised in continuity under local anaesthetic. Division of the stalk at its synovial origin is important in reducing the risk of recurrence, and is a standard component of our Treatments technique.

Excision carries a recurrence rate of approximately 10–20%, significantly lower than aspiration. It is the treatment of choice for recurrent ganglions, larger lesions, those causing neurological symptoms, and patients seeking definitive resolution.

Post-Procedure Recovery

Aspiration: Immediate return to activity, no dressings or sutures required.

Excision: A wound dressing is applied at the end of the procedure. A brief period of wrist splinting may be recommended for dorsal wrist ganglions to reduce post-operative discomfort. Sutures are removed within seven to fourteen days. Light activities are possible immediately; return to full manual activities within two to six weeks depending on site and occupation.

Observation Is Also a Valid Option

Not every ganglion requires treatment. Spontaneous resolution occurs in a meaningful proportion of cases, particularly in younger patients. If your ganglion is asymptomatic and not enlarging, a period of observation with planned review is clinically reasonable. We will give you an honest assessment of whether intervention is indicated in your case, and will never recommend treatment where it is not warranted.
