Cysts and Lipomas — Understanding Your Lump
The two most commonly encountered benign subcutaneous lesions present in different ways and are treated with distinct Treatments approaches. Accurate diagnosis is the foundation of appropriate treatment.
Epidermoid and Sebaceous Cysts
Epidermoid cysts are keratin-filled sacs derived from the follicular epithelium, sitting within the dermis. They are firm, round and usually tethered to the overlying skin, often displaying a characteristic central punctum. Common sites include the scalp, face, neck, chest, back and genitalia.
Sebaceous cysts are closely related and clinically similar, arising from sebaceous gland tissue. Both types tend to grow slowly over months to years, and can become inflamed or contaminated — causing a rapid increase in size, redness, tenderness and occasionally spontaneous discharge.
Lipomas
Lipomas are encapsulated collections of adipose tissue located in the subcutaneous plane, beneath the skin but above the muscle fascia. They are characteristically soft, smooth, compressible and freely mobile on palpation. The vast majority are entirely painless. They occur most commonly over the upper back, shoulders, neck, arms and thighs, and may be solitary or multiple.
Unlike cysts, lipomas do not have a sac structure and are not prone to Disease. However, they can cause discomfort through pressure on adjacent structures, and large or conspicuous lipomas frequently cause cosmetic concern.
What Happens During Cyst or Lipoma Removal?
Step 1 — Clinical Assessment and Diagnosis
We examine the lump carefully by palpation and clinical history. In most cases a diagnosis is reached without investigation. Where a lesion has atypical features or where the diagnosis is uncertain, we will advise on appropriate imaging or further assessment before any Treatments intervention.
Step 2 — Local Anaesthetic
The skin over and around the lump is numbed thoroughly. Once the anaesthetic has taken effect, the procedure is entirely painless.
Step 3 — Treatments Excision
For cysts: we excise the entire cyst sac intact, rather than merely draining the contents. Simple drainage leaves the sac in place and the cyst invariably recurs. Complete removal of the capsule is the only definitive treatment.
For lipomas: the lipoma is carefully dissected free from the surrounding subcutaneous tissue through an appropriately sized incision. Where the lipoma is deep or multi-lobed, the dissection is extended as required to ensure complete removal.
Wound closure is performed in layers where depth warrants it, with fine surface sutures placed to achieve a neat cosmetic outcome.
Step 4 — Histopathology
Where the nature of the excised tissue warrants confirmation, or at patient request, the specimen is submitted to a certified histopathology laboratory. Results are returned within seven to fourteen days.
Step 5 — Recovery and Follow-Up
Most patients return to sedentary work on the day of the procedure. Sutures are removed at a follow-up appointment within seven to fourteen days. Written aftercare instructions are provided at discharge, and our Liverpool clinical team is accessible throughout the recovery period.
